Notice to Parent

This release is intended to discharge in advance MHD

Elite Basketball and their agents, donor/sponsors and employees from and against any and all liability, claim, loss, damage, cost, or expense for negligent actions, or attributable in any way to, any negligent act or omission on the part of any such person or the MHD Elite Basketball arising out of or connected in any way with my athlete's participation in travel

basketball practice or games. I further understand that basketball involves physical contact between players, that serious accidents occasionally occur during such sporting activities, and that participants in such sporting activities occasionally sustain serious personal injuries (including death) and/or property damage, as a consequence thereof, knowing the risks of participation, nevertheless, I hereby agree to assume those risks on behalf of my son/daughter and release and hold harmless MHD Elite Basketball and their agents, sponsors, and/or employees who (though negligence or carelessness) might otherwise be liable to me (or my heirs or assigns) for damages.

Image Release

I hereby grant MHD Elite Basketball permission to use my child’s (the athlete/player’s) or my likeness in photographs, video recordings and/or electronic images in any and all of its publications, including website, and social media entries, without payment or any other consideration. I understand and agree that these materials will become the property of the

organization and will not be returned. I hereby irrevocably authorize the organization to edit, alter, copy, exhibit, publish

and distribute these images for purposes of publicizing the organization's programs or for any other lawful purpose. In

addition, I waive the right to inspect or approve the finished product, including written or electronic copy, wherein the

athlete/player’s or my likeness appears. Additionally, I waive any right to royalties or other compensation arising or related

to the use of my or my child’s (the athlete/player’s) image. I hereby hold harmless and release and forever discharge the

organization from all claims, demands, and/or causes of action which I, my heirs, representatives, executors, administrators, or any other persons acting on my behalf or on behalf of my estate have or may have by reason of this authorization.
